USAGE: 1 year (with the exception of Website and Social media which is in perpetuity); stills used for Brochure, Hospital TV, Billboard, Website and Social Media (NY and CT only)
RATE: $500/4 hrs, paid NET 30, no travel pay, no agency fee
SEEKING:

- include links to reels/clips in the body of email
- include current height, weight and location base in the body of email
