SUBMISSION INSTRUCTIONS (*DEADLINE FOR SUBMISSION – 9 AM Monday 10/26/15)

A)  COMPLETE AND SUBMIT REQUIRED INFO:

1. name/email/cell/city based
2. age and date of birth (minors only)
3. do you own a car to self-report to ALBANY, NY?
4. current vital stats (height/weight/hair & eye color)
5. agency/agent name/email/cell (if available)
6. all profile links (Actors Access, Backstage, Casting Networks, Website, etc) 
7. jpeg of CURRENT headshot/pics 
8. pdf of UPDATED resume
9. jpeg of CURRENT selfie to be submitted to client (MANDATORY-shoot from shoulders up clearly showing face and hair length, without photo effects and filters)
10. Adults- provide full URLs of your media links (reels/monologues).  
11. Kids- submit a personality clip if professional media links are not available.   Personality clip should be no longer than 2 mins, uploaded to Vimeo (preferred and can be password protected- make sure to provide password) or YouTube (not password protected), with full URLs sent to us.  No other formats will be accepted.
